Dear Sir or Ms:

As a service to professors and students here at UW-River Falls , our facility issues instructional resources at the beginning of each term and collects them after the close of the term. Generally, we purchase materials directly from publishers, however, in cases where the material is out of print, or only a portion of the material is needed, we ask permission to duplicate material in sufficient quantities to enable each student to have the information in the form of a course packet. There is a need for duplication of some pages of the work listed below. You are listed as the copyright holder.

The part of the material which we are interested in duplicating for our course entitled, (course title), professor (name) is:

with a possible class enrollment increase to (quantity).
Type of reprint: (offset, microfiche, disc, video, film, audio)

Also, for our students with visual disabilities, we are requesting that the Reading Taping Service at our University be permitted to transcribe the material onto a cassette tape.

Complete bibliographic credit will be given.

We would appreciate your reply to our request by (date) so that we can proceed with making arrangement for classroom instruction.

A self-addressed, stamped envelope and a copyright permission form are enclosed for your convenience. If you request a fee, please include your social security number or tax exempt number.
